Terms & Conditions To confirm your appointment, I kindly ask for full payment at the time of booking. This helps in two key ways:
For you: It secures your chosen time slot and allows you to fully commit to the session, knowing everything is taken care of in advance.
For me: It helps me manage my time and workload effectively, and ensures that I can continue offering flexible appointments and high-quality care to all clients.
1. Cancellation and Refunds (Self-Pay Patients)
Full payment is required at the time of booking.
Cancellations made within 24 hours of the appointment time are non-refundable.
Patients may reschedule once without charge, provided the new appointment takes place within 30 days of the original appointment date.
If the appointment is not rescheduled within this 30-day period, the original payment will be forfeited.
2. Required Forms and Documentation
Patients are required to complete and return all pre-appointment forms and questionnaires no later than 24 hours before the scheduled appointment.
If the required forms are not received within this timeframe, I reserve the right to cancel the appointment and offer an alternative date.
This policy ensures appointments run efficiently and allows appropriate preparation for safe and effective care.
3. Health Insurance Patients
Patients attending under a health insurance policy must follow their insurer’s rules for cancellations.
If a patient cancels and reschedules the same appointment twice in a row, I reserve the right to invoice the insurer for the appointment fee in accordance with provider terms.
If you are paying via a health insurance supplier like WPA or BUPA you will be sent a code to override the payment option within the booking form. You will need to receive this from me by email.
4. Late Attendance
If a patient arrives late, the appointment may be shortened to avoid impacting other scheduled clients.
If more than 15 minutes late, the appointment may be cancelled and classed as a missed appointment.
5. No-Show Policy
Failure to attend an appointment without prior notice will be treated as a cancellation within 24 hours, and the full fee will be retained. There may be circumstances where cancellation cannot be helped - cases can be discussed on an individual basis.
The FODMAP package sessions must be scheduled up front and taken within a 4 month period. This allows time for the elimination diet (no more than 4 weeks) and the reintroduction stage (up to 8 weeks) and the personalisation session to take place.

Consent for Amy Gittins to carry out a consultation. You will be contacted via phone or video call (depending upon your preference) and if one method hasn’t worked, a back-up method will be provided (telephone or alternative video call software).
Consent for Amy Gittins to contact you regarding your dietetic treatment. This involves follow up phone, text messages or emails as required. You consent to have reminders and resources sent to you.
Consent for Amy Gittins to contact your General Practitioner (GP) or other healthcare professionals, if required, as part of your dietetic treatment. This includes the sharing or obtaining of information regarding your care to improve your treatment. Prior to any communication, a discussion will take place during your consultation to obtain consent.
Consent for Amy Gittins to handle your data following GDPR guidelines for the purposes of providing dietetic treatment. This information includes personal contact details, medical history and current medical status and weight. You understand that Amy Gittins is required to hold your records for 7 years as per HCPC guidelines.
