Terms & ConditionsThank you for choosing Wholefoods1st. To secure your appointment, full payment is required at the time of booking (unless you are attending through an approved private health insurance provider). These terms are designed to ensure appointments run smoothly and that every patient receives the time, preparation and care they deserve.
1. Payment
Full payment is required at the time of booking for self-paying patients.
Appointments are not confirmed until payment has been received or, where applicable, your health insurance authorisation has been confirmed.
Fees include reasonable preparation before your appointment and routine clinical documentation following your consultation.
Requests for additional reports, letters, forms or documentation outside routine clinical care may incur an additional fee.
2. Cancellation and Rescheduling
Appointments cancelled with less than 24 hours' notice are non-refundable.
You may reschedule your appointment once without charge, provided the new appointment takes place within 30 days of the original appointment.
If the appointment is not rescheduled within 30 days, the original payment will be forfeited.
Once a consultation package has commenced, refunds cannot be provided for any unused sessions.
Where exceptional circumstances arise, these will be considered on an individual basis.
3. Consultation PackagesUnless otherwise stated, consultation packages must be completed within 6 months of the initial consultation. Packages specifically designed for longer treatment programmes may allow up to 8 months, where stated at the time of booking. Unused appointments remaining after the expiry date will be forfeited and are non-refundable. It is the patient's responsibility to arrange follow-up appointments within the package validity period. Low FODMAP PackageThe Low FODMAP package has been specifically designed to support patients through the elimination, reintroduction and personalisation phases of the diet. Appointments should normally be completed within 4 months, allowing sufficient time for:
Elimination phase (up to 4 weeks)
Reintroduction phase (up to 8 weeks)
Personalisation consultation
4. Pre-Appointment Forms and DocumentationTo ensure your consultation is productive and allows appropriate clinical preparation, all requested questionnaires, food diaries and supporting documentation must be returned at least 24 hours before your appointment. If the required information has not been received within this timeframe, I reserve the right to postpone your appointment until the documentation has been completed. Appointments postponed due to incomplete paperwork will be rearranged subject to availability.
5. Late AttendancePlease ensure you join your appointment at the scheduled time. If you arrive late, your consultation will still finish at the original scheduled end time to avoid delaying subsequent patients. Unfortunately, additional time cannot be added to the end of the consultation. If insufficient time remains to complete the consultation safely or effectively, I reserve the right to rearrange the appointment, which may be treated as a late cancellation.
6. Missed AppointmentsFailure to attend an appointment without prior notice will be treated as a late cancellation and the full consultation fee will be retained.
7. Private Health InsurancePatients attending under a private health insurance policy are responsible for ensuring they have valid authorisation before their appointment. Where applicable, you will be provided with a booking code to allow your appointment to be made without online payment. If your insurer does not cover the consultation fee, any outstanding balance remains your responsibility. Patients must also comply with the cancellation policies of their individual insurer.
8. Video and Telephone ConsultationsPlease ensure you have access to:
a reliable internet or telephone connection;
a quiet and private environment; and
a suitable device for the consultation.
If technical difficulties occur, reasonable efforts will be made to continue the consultation via telephone or an alternative secure platform where possible.
9. Consultation Recording and Clinical NotesTo allow me to focus fully on our discussion rather than taking extensive notes during the consultation, I may make a secure audio recording solely for the purpose of producing an accurate written clinical record using confidential transcription software. These recordings:
are used only to prepare your clinical notes;
are not shared with third parties except where required for secure transcription services operating under appropriate confidentiality and data protection standards;
are stored securely; and
are permanently deleted once the consultation has been transcribed and your clinical notes have been completed.
By booking an appointment, you consent to this process. If you would prefer your consultation not to be recorded in this way, please let me know before your appointment and I will instead take written notes during the consultation.
10. Clinical ResponsibilityDietetic advice is based upon the information you provide during your consultation. It is your responsibility to ensure that information relating to your medical history, medications, allergies, investigations and current symptoms is accurate and up to date. Recommendations are based on the information available at the time of consultation and may need to be reviewed if your health circumstances change.
11. Consent to Dietetic TreatmentBy booking an appointment with Amy Gittins at Wholefoods1st, you consent to the following:
Consultation with Amy Gittins via video consultation or telephone, depending upon your chosen appointment type.
Being contacted regarding your dietetic care by email, telephone or text message where appropriate, including appointment reminders, educational resources and follow-up information.
Communication with your GP or other healthcare professionals involved in your care where appropriate and with your consent, unless disclosure is required by law or safeguarding obligations.
The creation and storage of clinical records for the purposes of providing safe and effective dietetic care.
12. Confidentiality and Data ProtectionYour personal information will be processed in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. Information collected may include:
contact details;
medical history;
medications;
investigation results;
dietary information;
consultation notes;
weight and other relevant clinical information.
Your records are stored securely and retained in accordance with professional HCPC record retention requirements. Information will remain confidential unless:
you provide consent for it to be shared;
there is a legal obligation to disclose information; or
disclosure is necessary to protect you or another person from serious harm.

13. Circumstances Beyond Our ControlOccasionally appointments may need to be postponed due to illness, severe weather, technical failure or other circumstances beyond my reasonable control. If this happens, an alternative appointment will be offered as soon as reasonably possible.
Professional RegistrationAmy Gittins is a UK Registered Dietitian with the Health and Care Professions Council (HCPC) and a member of the British Dietetic Association (BDA). As a registered healthcare professional, Amy is required to practise in accordance with the HCPC Standards of Conduct, Performance and Ethics and the BDA Code of Professional Conduct.
